Accurate patient information is essential for correctly filing claims, generating patient schedules, physician's letters and account statements. When you call to schedule an appointment, you will be asked to provide registration information. Please have your insurance card available when calling for an appointment.
We will ask to make a copy of your insurance card at each visit. The card should include the name and address of your health insurance company, your policy number and the address where the claims are to be filed. If you do not have a valid insurance card, you will be asked for a deposit prior to receiving care. Additional deposits may be necessary after your initial consultation.
When you come in for your visit, it is always necessary to inform our front desk staff whenever there is a change in your health insurance coverage, mailing address, phone number and emergency contacts.
